Y.O.U.T.H. INITIATIVE GRANT APPLICATION
(Yumans' Obesity Undermines Their Health)
The YOUTH Initiative of the Foundation of Yuma Regional Medical Center provides a $100,000 annual fund to support local non-profit organizations that pursue goals to reduce obesity in a defined Yuma-area population. Basic guidelines include:
- No award will exceed $10,000.
- All awards are for one fiscal year (October 1 through September 30)
- Though funding of all programs and projects and their amounts are limited to one year, each is always competitively renewable.
- Preference will be given to projects/programs directed at youth, but will consider any that seriously demonstrate the high likelihood of inducing long-term healthy lifestyle changes.
Applications for the Y.O.U.T.H. Initiative grant funds will be reviewed quarterly by the YOUTH Initiative Committee. Allow 60 days for review and processing your application. The application should consist of a letter of introduction, copy of your IRS designation letter, the application form and a narrative not to exceed two single-spaced pages containing the following information:
- Detail the demonstrated need for the proposed project/program.
- A specific set of objectives that define the intended outcomes to be achieved within one year, as well as within five years should funding continue.
- Assurance the funds do not replace the funds of an existing project/program nor one discontinued within the previous twelve months. Monies may be awarded, however, to expand current project/program.
- An outline, with citation of relevant medical and/or research evidence suggesting that the proposed project/program has sufficient potential to meet the stated objectives.
- An annual budget that shows how the project/program objectives will be met.
- The names of all personnel who will conduct the project/program and their respective responsibilities as well as their qualifications for carrying out the responsibilities.
Award recipients must present a year-end report that cites data quantifying the extent to which the objectives of the application were met. It must also include at least one anonymous personal success story that can be shared with the community.
